The History of Breast Implants
Modern breast implants are the result of more than sixty years of innovation, research, and refinement. Every improvement in implant design has been driven by one goal: creating a breast that looks and feels more natural while improving long-term safety and durability.
Understanding this history helps explain why today’s implants are designed the way they are and why our surgeons make specific recommendations regarding implant selection.
The Early Years
Attempts to enlarge the breast date back more than a century. In the late 1800s and early 1900s, surgeons experimented with a variety of materials including paraffin, ivory, glass balls, rubber, cartilage, fat, and even sponges. While many of these approaches initially appeared promising, nearly all resulted in significant complications including infection, chronic inflammation, migration of the material, or severe breast deformity.
These early experiences taught an important lesson: the body reacts differently to different materials, and long-term biocompatibility is just as important as the initial cosmetic result.
The Birth of the Silicone Breast Implant
A major breakthrough occurred in 1962 when plastic surgeons Dr. Thomas Cronin and Dr. Frank Gerow, working with the Dow Corning Corporation, introduced the first silicone gel-filled breast implant. For the first time, surgeons had an implant that closely resembled the softness and feel of natural breast tissue.
The first-generation implants had thick silicone shells and were filled with a relatively firm silicone gel. Although durable, they also developed significant scar tissue around the implant, a condition known as capsular contracture.
As implant technology evolved through the 1970s and 1980s, manufacturers sought to create softer, more natural-feeling implants. Shells became thinner and the silicone gel became less cohesive. While these changes improved softness, they also increased the risk of silicone gel bleed and implant rupture.
The Saline Era
Saline-filled implants were introduced in France during the 1960s and became increasingly popular over the following decades. Because they are inserted empty and filled after placement, saline implants require smaller incisions and allow surgeons to make minor adjustments in implant volume during surgery.
Following concerns regarding silicone implants in the early 1990s, saline implants became the most commonly used breast implant in the United States. While they offered reassurance because the body safely absorbs sterile salt water if a rupture occurs, saline implants generally felt firmer and were more prone to visible rippling than silicone implants.
The Silicone Moratorium
One of the most significant events in the history of breast augmentation occurred in 1992, when the FDA placed a moratorium on the routine cosmetic use of silicone gel-filled breast implants. This decision was prompted by concerns that silicone implants might be associated with autoimmune diseases such as lupus, rheumatoid arthritis, or scleroderma.
Over the next fourteen years, researchers around the world conducted extensive scientific studies involving hundreds of thousands of women. Time and again, these studies failed to demonstrate a causal relationship between silicone breast implants and systemic autoimmune disease.
As a result of this growing body of evidence, the FDA reapproved silicone gel breast implants for cosmetic breast augmentation in 2006. The moratorium ultimately became one of the most extensively studied periods in the history of medical devices, leading to a far greater understanding of implant safety than existed before.
The Modern Era
Today’s fifth- and sixth-generation silicone implants are dramatically different from those used decades ago. Manufacturers have developed stronger shells, more durable construction, and highly cohesive silicone gels that better maintain their shape while reducing silicone migration should an implant rupture.
Modern implants are available in a variety of gel cohesivities, projections, widths, and profiles, allowing surgeons to tailor implant selection to each patient’s anatomy and desired aesthetic outcome.
Equally important, advances in surgical technique – including improved pocket dissection, meticulous infection prevention protocols, refined incision placement, and better understanding of breast anatomy – have significantly improved both patient safety and long-term results.
Looking Ahead
Breast implant technology continues to evolve. Newer implant designs focus on improving long-term durability, creating an even more natural feel, and optimizing how the implant interacts with surrounding tissues. Manufacturers continue to invest heavily in research, while surgeons continually refine surgical techniques to further reduce complications and improve outcomes.
